Job Summary:
Are you someone who finds joy in helping others and making a real difference every day? At PACE-RI, we’re looking for compassionate, reliable Home Care CNAs to join our team and support older adults in their homes. In this role, you’ll provide personal care based on individualized care plans—assisting with tasks like bathing, grooming, toileting, eating, and oral care—while helping to maintain a clean, safe, and comfortable home environment through light homemaking duties such as laundry, meal prep, and shopping.

You’ll also play a key role in providing behavioral support, monitoring for signs of agitation, wandering, or other behavioral changes, and reporting any emotional, physical, or cognitive shifts to our Home Care Department. Using equipment like gait belts or Hoyer lifts, you’ll follow safety procedures and care plans to support safe mobility and transfers. Prompt response to alarms and emergency situations is part of the role, as is ensuring that all care and interactions respect the participant’s dignity and privacy in line with our Participant Rights Policy.

Documentation is an important part of what we do—you’ll clock in and out at each visit, record completed care tasks in our system (AxisCare), and enter thorough, timely notes on any observations or changes in a participant’s condition. You may also assist with simple restorative exercises such as walking, implement recreational activities in the home, and monitor supply levels to ensure participants never run out of essential items.

Full-Time Schedule –
This is a full-time role with a set weekly schedule. Schedule details will be confirmed upon hire (please indicate availability when applying). If you are interested in full-time work and can commit to a consistent schedule, we’d love to hear from you

PACE operates out of four day centers: East Providence, Westerly, Newport, and Woonsocket. We have participants throughout Rhode Island and try to assign Home Care CNAs within their preferred geographic area; however location preferences cannot be guaranteed.

Current coverage needs are in Southern Rhode Island (Westerly, Exeter, Hopkinton), the Central I-95/Route 4 Corridor (Johnston, Coventry, North Kingstown), and Barrington.
