Overview::
The PRN Nurse Practitioner is responsible for counseling patients on a preventive checkup using a remote (telehealth) consultation platform. While the patient is remote, the NP's hosting the consultations will work in our Catapult Health office in Dallas, TX (Addison area). Catapult’s licensed Nurse Practitioners review each patient’s risk of diabetes, heart attack, and stroke. Additionally, our Nurse Practitioners screen for depression and make referrals based on those results. Catapult Health’s Nurse Practitioners also provide each patient with an individualized action plan based on their checkup results. How you will make an impact::
  • Perform private preventive assessments of our patients through Catapult’s video conference tool. This is your way to truly empower every patient to make lifestyle changes and improve their health!
  • Educate all patients on health risk status related to diabetes, heart attack, and stroke prevention.
  • Utilize Catapult’s exclusive Electronic Medical Record to document all consultations.
  • Identify risk, provide clinical consultation and create an individualized action plan based on each patient’s results. This action plan will be specific to each patient and their individual health path(s) going forward.
  • Assess depression status/risk of each patient and refer them to depression resources, based on the assessment of that patient.
  • Identify and refer each patient to employee carrier resources, based on each patient’s individual results.
  • Actively engage patients in creating an action plan for their future health. This is a way to help the patient own a plan for going forward that works for them and they’re comfortable owning!
  • Counsel each patient based on current clinical guidelines (AHA, ACC, etc.)
What you need to be successful::
  • Current TX RN license (Compact license required)
  • Current (unencumbered) TX state license as APRN
  • Must be currently licensed in at least 3 additional states as a Nurse Practitioner (in addition to Texas)
  • Must be willilng to beocme licensed in at least 3-5 additional states in a short timeframe in order to see patients in the various states that our clients are located in.
  • Current national certification as FNP, AGNP, or ANP
  • Must provide availability and work (when offered) a minimum of 20 hours week. (Operating days are Monday through Sunday).
  • Shifts normally begin as early as 7:00AM and end as late as 9:00PM - Monday through Friday; Saturday's and Sunday's 8am - 3pm
  • At least 1 year of experience practicing as a Nurse Practitioner (Primary Care or Internal Medicine)
  • Strong computer skills (Ability to type and interact with patient simultaneously)
  • Strong communication and interpersonal skills to consult patients and keep them actively engaged. You want the patient to feel comfortable with you, so the consult should feel like more of a conversation!
  • Ability to stand or sit; talk and hear by headset; use hands to type for the duration of each shift.
  • Bilingual communication skills in Spanish, a strong plus!
